What is the entire range of services (i.e. meals, housekeeping, transportation, personal assistance with day-to-day activities, protection over sight, wellness programs, medication management, recreational opportunities, etc.) that is residence routinely provides?

Which services are included in my base monthly fee, and which additional services must I pay for beyond those included in my monthly fee?

How are the supportive care services (i.e. assistance with bathing ambulating, toileting, dressing, eating, administration of medication, etc.) that I need identified?

How frequently will my health care status be evaluated to determine if my supportive care needs changed?

Who will conduct my evaluation and can my personal physician participate in this process?

What is your policy concerning the use of services provided by organizations that are not affiliated with the assisted living community, (i.e. home health care agencies, pharmacies, private duty personnel, etc.)?
